Radiology: Imaging Cancer - Pulsatile Inferior Vena Cava Contrast Material Reflux in Carcinoid Heart DiseaseMay 27, 2022Figure 1: CT image in a 67-year-old man who presented with a 2-year history of flushing and worsening diarrhea. Physical examination demonstrated an enlarged liver, and abdominal US showed multiple liver masses. Subsequent CT of the abdomen and pelvis with intravenous contrast material showed known hyperenhancing hepatic metastasis (arrowheads) and additional lesions in the small-bowel mesentery and terminal ileum (not shown). Reconstructed coronal images of the arterial phase showed reflux of contrast material into the hepatic inferior vena cava in a peculiar pattern of alternating bands of contrast-enhanced and unopacified blood (arrows). Based on symptoms and imaging findings, the diagnosis of metastatic neuroendocrine tumor with secondary tricuspid valve insufficiency was suggested. The patient had no prior cardiac history. There is slightly later enhancement of the basal segments of the left lower lobe being supplied by a large tortuous anomalous artery (A) from the descending aorta. The left lower lobe arteries drain into the enlarged left inferior pulmonary vein (V). A vein draining the superior segment of the left lower lobe (SV) demonstrates earlier enhancement than the remaining left lower lobe veins. There is absence of the left lower lobe basilar artery distal to the origin of the superior segmental branch (S) from the left pulmonary artery (P in B).
